  1. Coroner case #701408087
    On October 30, 2014, at 1100 AM, the California Highway Patrol emergency dispatch received a 911 call reporting a single vehicle accident had occurred on the Interstate 15, north of Alvord Mountain Road in Landers. Upon arrival, California Highway Patrol personnel found that 29 year-old Rosemead resident  Xiag-Xu Li had been traveling northbound on the Interstate 15 when for unknown reasons the vehicle  went into the center median. As the vehicle entered the center median it began to roll and caught fire when it came to rest. Li and his passenger were pulled from the vehicle by an off-duty Las Vegas Metro Police Officer. Li was pronounced dead on scene by an off-duty registered nurse who was assisting with life saving efforts. The passenger was transported to a medical center for treatment. The California Highway Patrol is investigating this incident. [103114 0732 SY]  


  2. Coroner case #701408040
    On 10/28/14 at about 5:00 PM  the Montclair Police Department received a 911 call reporting a shooting in the 4500 block of Canoga St. Officers and paramedics responded to find a 20 year old male had been shot by unknown suspects. He was pronounced dead at the scene. When positive identification is completed his name will be released to the public. Montclair Police Department is investigating this incident. [102914 0751 SY] 


  3. Coroner case #701408010
    On 10/27/2014 at 6:27 PM, a 911 call was received reporting a head on collision between a semi tractor trailer and a passenger car. 75 year-old Kingman, AZ resident Judith Ann Hernandez was the front seat passenger in a 2011 Ford Fusion which was traveling northbound on US Highway 95 S. In Vidal, at 0.2 mile south of mile marker 20, the Ford attempted to pass a northbound tractor trailer and collided with a southbound semi tractor trailer. Hernandez was pronounced dead at the scene. The California Highway Patrol is investigating this incident. [102814 1113 SY]

    On October 25, 2014, at 10:29 AM, the California Highway Patrol emergency dispatch received a 911 call reporting a single vehicle traffic incident had occurred on Mount Baldy Road near Ice House Canyon Road, Mount Baldy. Upon arrival, California Highway Patrol personnel found that a 1995 Chevrolet pick-up had been travelling southbound on Mount Baldy Road when it left the roadway. The vehicle continued down the mountainside rolling several times. 50 year-old Chino resident Hugh Geyer Hartman was pronounced dead at the scene by San Bernardino County Fire Department personnel. The California Highway Patrol is investigating this incident. [102814 1051 SY] 


  5. Coroner case #701407975
    On 10/17/14 at 10:07 PM Fontana Police received a 911 call reporting a single vehicle collision on Beech Ave., near Durango Way. Officers and paramedics arrived to find a 1997 Honda Civic had struck a tree. A passenger, 16 year old Fontana resident Jordan Nicholas Nababan, was transported to Arrowhead Regional Hospital where he died on 10/26/14. The Fontana Police Department is investigating the collision.[102814 0921 SY] 


  6. Coroner case #701407963
    On 10/25/2014, at 8:01 P.M.,  7 year-old Victorville resident Seth Wiley was the passenger in a white 1998 Saturn traveling north on Interstate 15 just north of State Route 138 when the Saturn was struck from rear by a black GMC 2500 pick up.  Wiley was transported to Desert Valley Hospital (DVH) where he later succumbed to his injuries. The California Highway Patrol is investigating this incident.[102814 0915 SY] 


  7. Coroner case #701407897
     On 10/23/2014 at approximately 0750 hours, 11 year-old Upland resident Isaiah Shelton was a pedestrian crossing the street at Benson Ave. and Arrow Route, in Upland. Shelton was struck by a vehicle which fled the scene after the collision. Shelton was transported to San Antonio Community Hospital where he was pronounced dead. The Upland Police Department and the San Bernardino County Sheriff-Coroner Division is investigating the death. [102414 0742 SY] 


  8. Coroner case #701407871
    On 10/22/14 at 5:18 AM, the California Highway Patrol began receiving calls of a head-on collision on Interstate 15 northbound just south of the Baseline Ave. exit in Rancho Cucamonga.  When officers arrived they found that a silver 2012 Toyota Prius  had been traveling southbound in the northbound lanes of traffic and struck a semi-tractor trailer that was traveling northbound in the lane closest to the shoulder.  The Prius suffered total damage as the result of the collision.  Paramedics from the Rancho Cucamonga Fire Department arrived shortly after receiving the call and pronounced the male 19 year-old Prius driver dead.  The name will be released following notification to next-of-kin. The Rancho Cucamonga Station of the San Bernardino County Sheriff's Department, Major Accident Investigation Team is investigating the incident.  [102214 1412 SY] Name and corrected age released: Liam Niemeyer, an 18 year old resident of Lancaster [102214 1649 SY] 


  9. Coroner case # 701407865
    On October 18, 2014, at 6:45 A.M., San Bernardino County Sheriff Department Personnel responded to a report of a single vehicle traffic incident on Caliente Road at Joshua Street in Hesperia. Sheriff personnel found 45 year-old Hesperia resident Tamara Sigmund, the sole occupant of a 2002 Chevrolet, had been traveling northbound on Caliente Road when for unknown reasons the vehicle rolled over multiple times. Sigmund was transported to the Arrowhead Regional Medical Center. On 10/21/14 at 10:57 P.M. died of her injuries. The San Bernardino County Sheriff Department, Major Accident Investigation Team is investigating the incident. [102214 1404 SY]
