  1. Coroner case #701704265
    On 06/20/17, at 7:03 AM, officers with San Bernardino Police Department and emergency medical service personnel, responded to a traffic collision on 5th Street, just west of Garner Ave, in the city of San Bernardino. Two men ran northbound across 5th Street and were struck by an eastbound traveling sedan. Paramedics found Thomas Carrasco, a 25-year-old male resident of San Bernardino, on the roadway with traumatic injuries. Paramedics pronounced Carrasco dead at 7:16 AM. San Bernardino Police Department is investigating the collision. [062617 1530 TC]

    On 06/25/17, at 6:41 AM, law enforcement and emergency medical service personnel responded to a 911 call of a single vehicle rollover collision on Tussing Ranch Road, just east of Mockingbird Avenue, in the unincorporated area of Apple Valley. Paramedics arrived and found the driver, David Felix, a 49-year-old male resident of Apple Valley, with traumatic injuries. He was pronounced dead at 6:53 AM. California Highway Patrol is investigating the circumstances of the collision. [062617 1530 TC] 


  3. Coroner Case # 701704409, 701704411
    On Friday, 06/23/2017, at 12:58 P.M., the California Highway Patrol (CHP) dispatch received a 911 call regarding a single vehicle rollover on the westbound Interstate 10, just east of Live Oak Canyon Road in Yucaipa. CHP officers and paramedics responded to the scene and found that two ejected occupants of the vehicle sustained various injuries. An adult Hispanic male, (later identified as 25 year old Ivan Flores if Glendale, AZ) and an adult Black female (later identified as 30 year old Cherrelle Malloy of Glendale, AZ) both died as a result of the collision. Malloy was pronounced dead on scene at 1:03 P.M., and Flores was pronounced dead at Loma Linda University Medical Center at 1:40 P.M. The CHP is investigating the incident. [06242017 0215 SC] 


  4. Coroner case #701704395
    On 06/22/2017 at 3:23 PM, the California Highway Patrol (CHP) received a wireless 911 call reporting a single vehicle collision on the westbound lanes of Interstate 40 about 8 miles east of Ludlow. Emergency crews responded to find Allan Bullard, a 47 year old resident of South Gate, had been ejected from his vehicle. Despite rescue efforts, Bullard was pronounced dead on scene at 4:37 PM. The Barstow office of the California Highway Patrol is investigating the collision. [06232017 1015 JK] 


  5. Coroner case #701704402
    On Friday, 06/23/2017, at 3:43 AM, officers with the California Highway Patrol (CHP) responded to Cedar Avenue, just north of Marygold Avenue in Fontana in reference to a pedestrian struck by vehicles. The pedestrian was transported to Arrowhead Regional Medical Center, where death was pronounced at 4:22 AM. His name will be released once identification and notification to his family are complete.   6. Coroner case #701704259
    On Friday evening, June 16, 2017, at 8:38 PM, Ryan Dennis Angus, a 47 year old resident of Hesperia, was riding a bicycle along Ranchero Rd. in Hesperia, when he was struck in the head from behind by the protruding side-view mirror of a passing vehicle. Angus was flown to Arrowhead Regional Medical Center, where he was pronounced dead the following morning, Saturday, June 17th, at 10:00 AM. The collision in under investigation by the California Highway Patrol. [06182017 0210 SC] 


  7. Coroner case #701704265
    On Saturday, June 17, 2017, at 9:58 PM, deputies with the San Bernardino County Sheriff's Department, officers with the California Highway Patrol, and paramedics with the San Bernardino County Fire Department responded to a single ATV collision at the El Mirage OHV area in Adelanto. Paramedics located, 22 year old Jesus Adagar of San Bernardino, with trauma. At 10:26 PM, Jesus was pronounced deceased at the scene. The California Highway Patrol is investigating the incident.   8. Coroner case #701704225
    On 06/15/17, at 11:40 PM, the California Highway Patrol responded to Interstate 15, north of Stoddard Wells Road, in Apple Valley, reference a two-vehicle traffic collision. Upon arrival, they found a Toyota pick-up, that had driven beneath the rear of a semi-tractor and trailer and burst into flames. The driver of the Toyota was declared dead on scene by San Bernardino County Fire Department personnel on June 16, 2017, at 12:01 AM. The name of the driver is being withheld pending identification. The California Highway Patrol- Victorville Office is investigating the incident. [061817 1550 TC] 


  9. Coroner case #701704226
    On 06/16/17, officers with the Rialto Police Department responded to a domestic violence incident in the 2800 block of W. Birch Street in Rialto. 30-year-old Rialto resident Hector Soria answered the door armed with a handgun. An officer-involved shooting occurred at 2:07 AM. Soria later died from his injuries at Arrowhead Regional Medical Center at 3:13 AM. Rialto Police are investigating the incident. [061817 1545 TC] 


  10. Coroner Case #701704252
    On Saturday, 06/14/17, at 4:42 AM Deputies with the San Bernardino County Sheriff’s Department, Town of Apple Valley station responded to Bear Valley Road near Reata Road for a traffic collision.  When they arrived they found the vehicle Phillip Kain, a 46 year old resident of Apple Valley was driving, struck the rear of a Freightliner tractor trailer. Paramedics with the Apple Valley Fire Department responded and confirmed Kain dead at 4:55 AM. San Bernardino County Sheriff’s Department, Apple Valley Major Accident Investigation Team is investigating the circumstances of the collision. [06172017 1830 JK]

    On Saturday, 06/17/17, at 9:50 AM, California Highway Patrol Officers responded to a traffic collision on northbound Interstate 15, just south of 1st Street, in Yermo. They found Jing Fan, a 61-year-old female resident of Rancho Cucamonga, was a passenger of a SUV that left the roadway and rolled off the Northbound Interstate 15 shoulder. Emergency medical service personnel responded and Paramedics pronounced Fan dead at the scene at 10:11 AM. California Highway Patrol is investigating the collision. [06172017 1830 JK] 


  12. Coroner Case # 701704247
    On Friday, 06/16/2017, at 10:25 P.M., the San Bernardino County Sheriff's Department (SBSD) dispatch received a 911 call after an unidentified adult female was struck by an oncoming vehicle in the area of Hesperia Road and Comanche Road in Victorville. The pedestrian was transported by paramedics to Desert Valley Hospital with lifesaving measures being performed while en route. Upon their arrival at the Emergency Department, the pedestrian was pronounced dead at 11:23 P.M. The SBSD is investigating the collision. The female's identity will be released after she has been identified and her family has been notified of the death. [06172017 0548 SC]

    On Wednesday, 06/14/2017, at 9:59 P.M., the California Highway Patrol dispatch received a 911 call after an unidentified adult White male was struck by multiple oncoming vehicles while walking on the northbound lanes of Interstate 215 between 5th and Baseline St, in San Bernardino. Paramedics and CHP officers responded to the scene and pronounced the male dead upon arrival at 10:22 P.M. The CHP is investigating the incident. The male was later identified as 27 year old, Andrew Miles, of Waco, TX.. [06172017 1830 JK] 


  14. Coroner case #701704213
    On Thursday, June 15, 2017, at 3:49 PM, a collision was reported along the westbound Oak Glen Rd., just two-tenths of a mile east of the Interstate 10 Freeway. A gray 2003 Toyota Tundra pick-up truck traveling at a high rate of speed skidded into the curb off the right (north) and struck a tree. Paramedics from CAL-Fire arrived and pronounced the driver, and sole occupant, 41 year old Craig William Hanley of Yucaipa, dead at 3:56 PM. The accident is under investigation by the Sheriff Department, Major Accident Investigation Team (MAIT). [06162017 0325 SC] 


  15. Coroner case #701704204
    On Thursday, June 15, 2017, at about 11:00 AM, the body of Deatrice Yunvondia Frierson, a 55 year old resident of Coachella (Riverside County), was found in the open desert east of Singing Sands Rd., a quarter mile north of the Twenty-Nine Palms Highway in Twenty-Nine Palms. The decedent’s car was found about .2 of a mile to the east, stuck in soft sand. Examinations will be conducted in an attempt to determine the cause of death.   16. Coroner Case #701704206
    On Thursday, 06/15/2017, at 11:35am, 15 year old Moises Chequete, a resident of Hesperia, was struck from behind by a vehicle as he walked northbound in a designated bicycle path on Santa Fe Avenue, north of Mission Street. Moises was transported by ambulance to Desert Valley Hospital where he was pronounced dead from his injuries in the emergency room. Hesperia Sheriff's Station is investigating this collision.   17. Coroner Case #701701203
    On Thursday, 06/14/2017, at 9:08am, 25 year old Muataz Abdelatif, a resident of New Jersey, was the driver and sole occupant traveling eastbound on Palmdale Road west of Highway 395 when he crossed the double yellow line and struck a westbound semi-truck head on. Muataz was transported by ambulance to Victor Valley Global Medical Center where he was pronounced dead in the emergency room. Adelanto Sheriff's Station is investigating this traffic collision.     On Wednesday, 06/14/2017, at 9:59 P.M., the California Highway Patrol dispatch received a 911 call after an unidentified (possible middle aged) adult Hispanic or White male was struck by multiple oncoming vehicles while walking on the northbound lanes of Interstate 215. Paramedics and CHP officers responded to the scene and pronounced the male dead on scene at 10:22 P.M. The CHP is investigating the incident, and the decedent's identity will be released after he has been identified and his family has been notified. [06152017 0320 SC]

    Coroner Case # 701704189: On Wednesday, 06/14/2017, at 5:07 P.M., the San Bernardino Police Department (SBPD) dispatch received a 911 call after an adult White male was found unresponsive on a picnic table at Wildwood Park in San Bernardino. Paramedics responded to the scene and found the male with obvious signs of death and confirmed him dead at the scene at 5:16 P.M. The SBPD is investigating the incident. UPDATE: The male was later identified as 41 year old, Jason Cable of the San Bernardino area. [06182017 0215 SC] 


  20. Coroner Case #701704147
    On Monday, 06/12/2017, at 6:05 pm, deputies from the San Bernardino County Sheriff’s Department responded to a traffic collision in the 13400 block of Mariposa Rd., in Victorville. When deputies and paramedics arrived, they found a 2007 Honda Accord had been northbound Mariposa Road when the driver, Sergio Fabian Martinez, age: 21 year-old, from Victorville, lost control of the vehicle causing it to strike a light pole and a fire hydrant before rolling over onto the sidewalk. Paramedics pronounced him dead at the scene. The San Bernardino County Sheriff’s Department Victorville Station is investigating the collision. [06132017 0350 EM] 


  21. Coroner case# 701704160
    On 06/08/17, at 4:58 PM, deputies and paramedics responded to a "shots fired" call in the 1400 block of N. Sepulveda Avenue in San Bernardino. 19 year-old San Bernardino resident Oland Mayweather was found with trauma to his upper body inside an apartment. Mayweather later expired at Arrowhead Regional Medical Center (ARMC) on 06/12/17 at 07:42 PM. San Bernardino Sheriff Homicide Detail is investigating the death. [061317 1336 TC] 


  22. Coroner Case 701703976
    On Monday, 06/05/2017, at 9:10 am, Colton Police officers responded to a 911 call in the 1400 Block of E. Santo Antonio Drive in Colton. When officers arrived on scene they found Damani Trouter, an 18 month old female unresponsive in an apartment. Paramedics arrived on scene and confirmed the death. Anyone with information is urged to contact the Colton Police Department at 909-370-5000.  An autopsy will be performed to determine the cause of death. [06062017 0210 EM]  


  23. Coroner case 701703939
    On Sunday, June 04, 2017, at 3:06 AM, the California Highway Patrol responded to a wrong way driver that struck multiple vehicles on the eastbound Interstate Highway 10, just east of Etiwanda Avenue in Fontana. The driver of one of the vehicles that was struck was a 38 year old Hispanic Female, possibly from Hemet. She was found to have traumatic injuries and was transported to Arrowhead Regional Medical Center where she was pronounced dead at 4:03 AM. When next of kin has been located and notified, her name will be released. The California Highway Patrol is investigating the incident. [06052017 0040] 


  24. Coroner Case # 701703938
    On Sunday, 06/04/2017, at 12:46 A.M., the San Bernardino County Sheriff's Department (SBSD) dispatch received a 911 call regarding a two vehicle traffic collision which occurred at the intersection of Hellman Avenue and E. 8th Street in Rancho Cucamonga. The driver of a Nissan Maxima was traveling southbound on Hellman Avenue and struck a Chevrolet Silverado which was traveling eastbound through the intersection. The truck sustained major damage and a passenger of the vehicle was ejected and later pronounced dead on scene by paramedics at 12:53 A.M. The decedent was identified as Abel Rivera, 59 years old of Fontana. The SBSD is investigating the collision. [06042017 2355 EM]
