Biometric Screening and online Health Assessment Rewards Campaign
Biometric Screening Event Dates: Biometric screening events will be held throughout the County. Blue Shield will be conducting biometric screenings for Blue Shield subscribers by appointment, and Kaiser Permanente subscribers will be available to assist Kaiser members with health portal registration. Blue Shield Subscribers
- Step 1: Register for the program and complete an online Well-Being Assessment by visiting www.mywellvolution.com.
- Step 2: Complete a Biometric Screening using one of the following methods:
- Register with Summit Health here to attend and complete your screening at an onsite biometric screening event. View the complete instructions.
- Visit a Quest Laboratory and have your complimentary screening completed. View the complete instructions.
Call your doctor to request a lipid panel with blood glucose screening. Download the Health Care Provider Biometric Screening form, and bring it with you to you to your exam. Then, have your doctor complete and sign the form and fax it to Summit Health at (248) 416-1197. The form may also be emailed to BSCAfaxes@summithealth.com.

Fight the flu! Get vaccinated! MHM! will once again partner with Arrowhead Regional Medical Center to provide free Flu Vaccines to all County employees during select Biometric Events throughout the 2016-17 Wellness Campaign. No appointment is necessary to receive a flu vaccine. Vaccines are administered on a first-come, first-served basis. Preservative-free vaccines will also be available upon request. NOTE: ARMC County Employees are advised to receive their flu vaccine at the ARMC Employee Health Department (lower level next to Arrowhead Café) in order to obtain proof of vaccination. For more information, call (909) 580-0084.
