Biometric Screening and Online Health Assessment Rewards Campaign
Biometric Screening Event Dates: Biometric screening events will be held throughout the County. Blue Shield will be conducting biometric screenings for Blue Shield subscribers by appointment, and Kaiser Permanente subscribers will be available to assist Kaiser subscribers with health portal registration. Blue Shield Subscribers
Know your numbers and earn a $50.00 reward gift card for completing a biometric screening between September 1st, 2017 through April 30th, 2018, using one of the following methods:
- In-Person: Complete your biometric screening at one of the onsite biometric screening events and receive your reward card at the event. Lost or stolen reward cards received at an onsite event cannot be replaced. Register to attend a scheduled event at www.cloviwell.com/programs/pgm_0bbd81#/onsite.
- Doctor's Appointment: Complete your biometric screening with your provider and submit the Physician Health Screening Form to OnSite Wellness via fax at (310) 618-1996 or email to Sang.Kim@onsitewellness.com with ‘My Health Matters!’ in the Subject Line. Once your completed form has been received and verified, a reward card will be mailed to your current mailing address. Please allow 3-4 weeks for delivery.

Kaiser Permanente Subscribers
Know your numbers and your health risks to earn a $50.00 reward gift card for completing all of the following required activities between September 1st, 2017 through April 30th, 2018:
- Activity 1: Register at www.kp.org/engage, accept the Wellness Program Agreement to be eligible to earn rewards, and take the online Total Health Assessment to get a wellness action plan.
- Activity 2: Complete or be current with your biometric screenings. To complete your Biometric Screening, take the Kaiser Biometric Screening Request Form to a Kaiser Permanente medical facility laboratory.

NOTE: Once registered on the rewards site, you will be asked to agree to participate in the wellness program to be eligible for rewards. If you do not accept the HIPAA agreement you can still use the site, but you will not be eligible for a reward.

Walkadoo Rewards Challenge - Coming January 12, 2018!
Walkadoo is a wireless pedometer-based program that makes walking fun! Each day, you get a step card via email or SMS that's been created just for you, based on your walking habits. Wear your pedometer, take your steps, and connect with other members on the Walkadoo website.

Fight the flu! Get vaccinated!
Blue Shield Members: Contact your provider to obtain your flu shot as part of your annual exam.
Kaiser Members: Get your flu shot today! As a Kaiser member, you can get a flu shot at no cost at our facilities or our flu shot clinics. To find a flu shot location or for more information, visit kp.org/flu.
NOTE: ARMC County Employees are advised to receive their flu vaccine at the ARMC Employee Health Department (lower level next to Arrowhead Café) in order to obtain proof of vaccination. For more information, call (909) 580-0084.

Eligibility for rewards: All rewards and incentives are subject to inventory/fund availability. Employees who are enrolled as a subscriber in a County-sponsored medical/dental plan may be eligible for rewards. Dependents may participate in MHM!, but are not eligible for rewards. Employees who opt-out or waive their health coverage to a County-employed spouse or registered domestic partner are not eligible for rewards.
