Forms and Resources
Eligibility for this insurance is governed by your Memorandum of Understanding (MOU), Exempt Compensation Plan, salary ordinance, or contract (employee-only coverage). For more information on the plan, please refer to the
Certificate of Insurance.
Note: If you are on a leave of absence and have not coded enough hours to meet the eligibility requirements for the County to pay your basic life insurance premium, you have the option to continue this benefit at your own expense.
Insurance benefits will be automatically paid to your beneficiary in the following order:
- Surviving spouse/domestic partner
- Surviving children
- Your parents
- Your siblings
- Your estate
If more than one beneficiary becomes entitled to your benefits, they will share the benefits equally. To change the automatic beneficiary arrangement, you must complete the
Beneficiary Designation Form. If you elect to designate a beneficiary other than your spouse/domestic partner, you must have his or her written consent.
